School Vaccine Requirements

Parents please note that proof of vaccines is required at school enrollment or registration, and should be updated each time your child receives an immunization.

Tdap Will Be Required For Students Entering 6th through 12th Grade
Important note: 6th, 7th 8th, 9th, 10th, 11th, and 12th graders are required to have a single dose of Tdap.

Two Doses Of Varicella Vaccine Are Required For Children Entering Kindergarten, 1st through 6th Grade
The first vaccine must be administered no more than 4 days before the first birthday. There should be at least 3 months between the first and second dose if the child is under 13 years old, however if the second dose was given at least 28 days after the first dose, it does not need to be repeated. (If a health care provider documents that the child has had chickenpox disease, or if the child has lab confirmed immunity to the disease, the child has met the requirement and does not need the varicella vaccine.)

One varicella/chickenpox shot or disease history is required for grades 5 through 12.

Meningitis Vaccine
The vaccine is now recommended for all children 11-18 years of age, with a second dose recommended at age 16 through 18.  There is no need to revaccinate adolescents first vaccinated at 16 years or older.
